Obtaining the Application Form of UPCMET: The application form and information brochure of UPCMET can be acquired from the main branches of Oriental Bank of Commerce on cash payment of Rs. 1000/.

The application form of UPCMET ca also be obtained by sending a demand draft of Rs. 1050/ drawn in favor of UPUMCWA, Bareilly, U.P. along with self- addressed stickers to the Coordinator UPCMET, 1st floor Anand Ashram Complex, Rampur Garden, Bareily U.P. on or before 15th March.

Submitting the Application Form: Duly filled in application form should be sent by either registered post or speed post to the address given in the information brochure latest by 31st March.
